Whitewashed stone memorial with four sides, surmounted by a tiered pyramid topped with a decorated fleur de lis finial. Memorial sits on a square plinth on a wide square base with shallow iron railings painted green fixed to top of base.

Lewis Evan (1719-1792) was born in Trefeglwys but moved to Llanllugan when he was 18. He became the first Methodist Counsellor in North Wales and was a well known preacher and exhorter of the Calvinistic Methodists.He died soon after the completion of Capel Adfa and is buried in Llanllugan churchyard.
